간질(뇌전증) 치료 기본, Epilepsy basic treatment

1. 환아가 간질로 경련을 하는지 간질 이외 병으로 경련 하는 진단한다.
2. 적절한 간질 약을 선택, 치료용량을 알아본다.
3. 급성 간질지속 상태를 치료 한다.
4. 필요에 따른 응급 간질을 치료 한다.
5. 간질이 있는 여성의 임신에 관해 치료 한다.
6. 신경내과전문의. 정신과전문의, 단골 의사와 밀접한 관계를 가지고 치료한다.
7. 다음 진단 방법으로 확진 한다

 1) EEG( 뇌파 검사) 한다.

 2) 장기 비디오 뇌파 검사를 한다.

 3) MRI 검사를 한다.

(1)보통 MRI 검사를 한다.

(2) 3T MRI 검사를 한다.

(3) SPECT 또는 PET 검사를 한다.

4) 신경 정신과 검사를 한다.

5) WADA Test (혈관 조영상 검사)를 한다.

6) 피질 뇌파 검사 모니터링을 한다.

7)입체 뇌파 검사(SEEG)를 한다.

8. 다음과 같이 치료 한다.

1) 약으로 치료 한다

(1) 제일 세대 간질 치료약으로 치료 한다.

(2)세 세대 간질 치료약으로 치료 한다.

(3) 실험적 약으로 치료 한다

2) 미주 신경 자극으로 치료한다.

3) 뇌 수술로 치료 한다

  (1) 뇌엽절제술(Lobectomy)로 치료한다.

  (2) 환부 절제술로 치료 한다.

  (3) 뇌 환부를 절제 할 수 없으면 뇌 환부를 연결하는 부분을 절제술을 해 치료한다.

  (4)뇌량 절제술로 치료 한다.

  (5)기능적 뇌 반구 절제술로 치료 한다.

4) 음식물로 치료 한다

  (1) 케토젠닉 음식물로 치료 한다

  (2) 저 당뇨 인덱스로 치료한다.

  (3) 변형 Atkins 식사로 치료한다.

  • 간질의 종류와 원인 등에 따라 간질을 치료한다.
  • 부분 발작에 따라 또는 전신 발작에 따라, 간질의 원인에 따라 치료한다.
  • 그 원인에 따라 치료를 다르게 할 수 있다.
    • 대부분의 간질은 간질을 치료하는 약물로 적절히 치료하면 완치되거나
    • 일부의 간질은 완전히 치료가 되지 안 해도 간질 치료 약물로 계속 치료받는 중 간질이 발작되지 않고
    • 적절한 간질 치료 약물로 치료해도 일부 간질은 간질 발작이 가끔 유발될 수 있다. 
    • 간질의 극소수는 완치되지 않고 30∼50세까지 간질이 발작될 수 있다. 
    • 가끔 발작되는 간질을 15∼23년 동안 적절히 간질 치료 약물로 간질을 치료한 결과, 그 중 75%의 간질이 더 이상 발작되지 않았고 간질 약물치료를 더 이상 계속 하지 않았다는 연구도 있다. 
    • 간질을 적절히 간질치료약물로 치료하면 간질이 있는 아이들의 대부분이 간질이 없는 건강한 아이들과 똑같이 학교에 다닐 수 있고 정상적으로 성장 발육할 수 있고 건강한 성인들이 될 수 있다. 
    • 간질이 있는 아이들의 지능지수는 간질의 원인에 따라 다를 수 있으나 일반적으로 간질이 없는 건강한 아이들의 지능지수와 별로 다르지 않다. 간질 발작을 한다고 해서 지능지수가 낮아지지는 않는다. 

Epilepsy basic treatment

1. Diagnose whether the child has seizures due to epilepsy or seizures due to a disease other than epilepsy.

2. Select an appropriate epilepsy drug and find out the therapeutic dose.

3. Treat acute persistent epilepsy.

4. Treat emergency epilepsy as needed.

5. Treating pregnant women with epilepsy.

6. Neurosurgeon. He treats with a close relationship with a psychiatrist and regular doctor. 7. Confirm with the following diagnostic method

1) EEG (electroencephalography) is performed.

2) Do a long-term video EEG test.

3) Perform an MRI scan.

(1) Usually, MRI is performed.

(2) 3T MRI scan.

(3) SPECT or PET examination.

4) Do a neuropsychiatric examination.

5) Do the WADA Test (angiography test).

6) Monitoring of cortical EEG.

7) Perform a three-dimensional electroencephalogram (SEEG).

8. Treat as follows.

1) Treat with medication

(1) We treat with first-generation antiepileptic drugs.

(2) Treated with anti-epileptic drugs of the third generation.

(3) Treatment with experimental drugs

2) Treated with vagus nerve stimulation.

3) Treated with brain surgery

(1) It is treated with lobectomy.

(2) It is treated by resection of the affected area.

(3) If the brain affected area cannot be resected, the part connecting the brain affected area is treated by resection.

(4) Treated by corpus callosum resection.

(5) Treated with functional brain hemisphere resection

 4) Treat with food

(1) Treat with ketogenic food

(2) Treat with a low diabetes index.

(3) Treated with a modified Atkins diet.

(4) Conduct behavioral therapy.

• Treat epilepsy according to the type and cause of epilepsy.

• Treat according to partial seizures or generalized seizures, depending on the cause of epilepsy.

• Treatment may vary depending on the cause.

• o Most epilepsy can be cured or cured if properly treated with drugs to treat epilepsy. o Even if some epilepsy is not completely cured, epilepsy does not occur while continuing treatment with antiepileptic drugs.

o Some epilepsy may occasionally cause epileptic seizures, even when treated with appropriate antiepileptic medications.

o Very few cases of epilepsy are incurable and seizures may occur by the age of 30 to 50 years. Occasional epilepsy There is also a study showing that after 15 to 23 years of adequately treating epilepsy with antiepileptic drugs, 75% of them no longer have epilepsy and no further epilepsy medication is continued.

o When epilepsy is properly treated with antiepileptic drugs, most children with epilepsy can attend school, develop normally, and become healthy adults just like healthy children without epilepsy.

o The IQ of children with epilepsy may vary depending on the cause of the epilepsy, but in general it is not very different from the IQ of healthy children without epilepsy. Having an epileptic seizure does not lower your IQ.
