자폐증의 개요

  • 부모를 비롯해 다른 사람들과 사교성 상호관계를 갖지 않거나 정상적으로 갖는 데 지장이 있고, 
  • 언어발육이 지연되거나 발육되지 않고, 
  • 말로 하는 언어(구어)나 신체적 언어로 상호 대화를 하는데 심한 언어 장애가 있거나 대화를 정상적으로 할 수 없고, 때로는 언어발육이 상당히 지연되고, 말을 반복적으로 하기도 하고 판에 박은 반복적 행동을 하기도하고, 
  • 부모한테도 잘 안기지도 않을 수 있다. 
  • 그들의 IQ가 상당히 높을 수도 있고 낮을 수도 있다. 
  • 그러나 대부분의 자폐증을 가진 아이들의 IQ는 70이거나 그 이하이다. 
  • 이런 여러 증상 징후가 3세 이전 영유아들에게 나타는 것이 보통이다.
  • 이런 여러 증상 징후가 있는 복합적 신경발육 장애를 영아 자폐증, 소아 자폐증, 자폐증 스펙트럼 장애, 또는 자폐증이라고 한다. 
  • 3세 이전 영유아들에게 나타나는 것이 보통이다.
  • 자폐증을 자폐증 스펙트럼이란 병으로 부르기로 최근 정했다.

자폐증의 분류 
① 칸너 자폐증(Kanner’s Autism), 
② 애스퍼갤 자폐증(Asperger’s autism/Asperger disorder), 
③ 칸너 자폐증에도 속하지 않고 애스퍼갤 자폐증에도 속하지 않는 비정형적인 영아 자폐증 
④ 자폐증 스펙트럼 장애(Autism spectrum disorders) 
⑤ 레터 장애(Rett’s disorder) 

영아 자폐증(자폐증)과 자폐증 스펙트럼 장애의 원인

  • 원인은 아직도 확실히 모른다. 
  • 신경 생리 이상으로 자폐증이 생긴다는 학설도 있다. 
  • 뇌 손상, 대사 이상, 저능, 귀머거리, 정서 이상, 염색체 이상, 어떤 유전병으로 발병되기도 한다. 
  • 예방접종 후유증, 특히 MMR 예방접종백신 등으로 생긴다는 설도 있었지만 이 설을 확실히 뒷받침할 수 있는 근거가 아직 없다. 
  • 최근에는 발병률이 증가 추세이다. 
  • 소아 10,000명 중 30~50명에게 자폐증이 발견된다고 한다. 1,000명 중 3.3~10.6명의 소아들에게 자폐증 스펙트럼 장애가 발생됐다.(Journal watch, Pediatrics and Adolescent Medicine, May, 2007). 
  • 다시 설명하면, 자폐증은 칸너 자폐증(Kanner’s Autism)과 애스퍼갤 자폐증(Asperger’s autism), 칸너 자폐증에도 속하지 않고 애스퍼갤 자폐증에도 속하지 않는 비정형적인 영아 자폐증으로 분류된다. 
  •  요즘, 자폐증 스펙트럼 장애(Autism spectrum disorders), 레터장애도 자폐증 유형에 속하기도 한다. 
  • 407,500명의 소아들 중 2,685(0.66%)명에게 자폐증 스펙트럼 장애가 있었다는 연구도 있다. 
  • 최근 미 소아들의 150명 중 1명에게 자폐증 스펙트럼 장애가 있다. 
  •  8세 소아 44 명 중  1명의 소아가 자폐증 스펙트럼 장애를 가진다는 연구도 있다. 소스: 1/2022, AAP Newsd

미 소아청소년과 학회에서도 생후 18개월부터 24개월까지 정기 건강검진을 할 때 자폐증 스펙트럼 장애가 있는지 기본적으로 체크하도록 권장하고 있다.

  • 미국 소아들의 150명 중 1명에게 자폐증 스펙트럼 장애아가 있다.
  • 그래서 미 소아청소년과 학회에서는 생후 18개월에서 24개월에 정기 건강 검진을 소아청소년과에서 받을 때 기본적으로 자폐증 스펙트럼 장애아가 있는지 체크하라고 권장한다.
  • 특히 친 형제자매들 중 자폐증 스펙트럼 장애아가 있거나, 부모가 검진 받는 자녀에게 자폐증 스펙트럼 장애가 있는지 의심하거나, 그 유아를 돌보는 사람이 그 유아에게 자폐증 스펙트럼 장애가 있는 것 같다고 의심하거나, 검진해주는 단골 의사 자신이 자폐증 스펙트럼 장애가 있는 것 같다고 의심하면 유아 자폐증 진단 일람표(M-CHAT:Modified Checklist for Autism in Toddlers)로 자폐증이 있는지 없는지 스크린 해야 한다.
  •  자폐증이 있는지 없는지 스크린 진찰을  받은 경우도  확실한 자페증이 있다고  진단 받는 나이는 평균 4세이다. 소스: AAP News  8/2020.

  • 30년 전 소아 2,500명중 1명에게 소아 자폐증이 발생됐다. 그러나 최근 CDC의 연구결과에 의하면 소아 150명중에 1명에게 자폐증이 발생된다. 
  • 자폐증의 원인은 아직까지 확실히 규명되지 않았다. 
  • 유전적, 감염병, 환경조건 등이 자폐증의 원인이 되나 연구했으나 아직도 확실히 모른다고 이미 언급했다.
  • 자폐증이 얼마나 있는 지 적극적으로 확인하는 방법을 동원하고 자폐증을 진단하는 기준을 바꾼 후 자폐증 발병빈도가 증가되었다고 믿고 실제적으로 자폐증 발병빈도가 증가되고 있다는 사실도 배제할 수 없다. 
  • 감수량(減水量)이 증가되면 자폐증에 걸릴 위험성이 유전적으로 높은 아이들에게 자폐증이 유발될 수 있다는 연구결과가 나왔다(Archives of Pediatrics & Adolescent Medicine November 2008).더 많은 연구가 필요한 것 같다.

자폐증의 증상 징후

  • 증상 징후가 자폐증의 종류에 따라 많이 다르다. 
  • 칸너 자폐증(Kanner’s Autism)과 애스퍼갤 자폐증의 증상 징후의 대부분은 공통적이다. 
  • 징후 증상이 일률적으로 나타나지 않는다(다음 진단 참조). 주 증상들은 다음과 같다.
  • 아주 심한 장애가 사교적 관계에 있다. 
  • 눈길 접촉이 아주 적거나 없다. 
  • 판에 박은 동일 행동과 말을 반복한다. 
  • 다른 사람이 자기가 하는 일을 변경시키는 것을 아주 좋아하지 않는다.
  • 보통이 아닌 특이한 것에 흥미를 갖는다. 
  • 자폐증의 증상 징후가 날이 갈수록 조금씩 좋아질 수 있으나 환상은 생기지 않는다.
  • 출생 이후부터 조기 발육 이상이 생길 수 있다. 
  • 정신 분열증을 가진 아이들의 성장발육은 출생 후 얼마동안은 정상적이고 때로는 환상이 생기는 것이 보통이다. 이런 증상은 자폐증과 소아 정신 분열증을 감별 진단하는데 도움이 된다. 
  • 칸너 자폐증이 있는 아이들의 언어발육이 지연되거나 언어발육이 되지 않는 것이 보통이다.
  • 애스퍼갤 자폐증이 있는 아이들의 조기 언어발육은 임상적으로 현저히 지연되지 않는 것이 보통이다. 
  • 칸너 자폐증이 있는 아이들의 전체 신체 운동 발육이 현저히 지연되지는 않으나 때로는 전체 신체 운동이 어설프다. 
  • 그러나 애스퍼갤 자폐증이 있는 아이들의 미세 신체 운동도 전체 신체 운동에도 어설픈 점이 더 많이 있다. 
  • 칸너 자폐증이 있는 아이들의 75~80%에서 정신 박약증이 있고 애스퍼갤 자폐증이 있는 아이들에게는 정신 박약증이 덜 하다. 
  • 칸너 자폐증이 있는 아이들의 대부분은 기억력은 좋으나 애스퍼갤 자폐증이 있는 일부의 아이들의 기억력은 때로는 특수하게 좋은 면도 있다. 
  • 칸너 자폐증이 있는 아이들의 대부분에서 강박관렴이 강하고 반복적인 행동을 하고 반향 언어증이 있고 대화에 장애가 있고 경련 질환, 과도 활동 장애와 주의력 결핍 장애, 식이 패턴 이상이 있고 혼자 있기를 좋아하고, 똑같은 것을 계속 유지하려는 성질이 있고 자극에 과민하게 반응하고 인식적인 면은 좋다. 보통 지식층 가족의 아이들에게 이병이 생기는 것이 보통이다. 
  • 애스퍼갤 자폐증이 있는 아이들의 대부분에서, 사교적 상호작용 장애가 있고 남아들에게 더 잘 생기고 보통 영아기는 정상으로 발육되고 때로는 3세까지 정상으로 발육되기도 한다. 
  • 자폐증을 가진 아이는 정상 아이들과 다음과 같이 다른 점이 많이 있다. 
  • 자폐증이 있는 아이들은 부모나 부모 이 외 사람들과 친밀한 인간관계를 전혀 가지려고 하지 않거나 조금 밖에 갖지 않기 때문에 외롭게 혼자 있는 것이 보통이다. 
  • 공연히 빙빙 돌거나 방이나 마루바닥 등에 머리를 부딪치는 머리 부딪치기를 할 수 있고, 앉거나 서서 몸을 앞뒤로 흔들거나 밤낮 가리지 않고 이를 가는 버릇도 있다. p.00 머리 부딪치기 참조
  • 자폐증이 있는 아이는 방에 혼자 있을 때 부모나 누구든지 그의 방에 들어오든 말든, 말을 건네도 들은 척 만 척 하고, 오히려 듣기 싫어하기도 한다. 
  • 부모가 무릎 위에 앉으라고 손을 뻗쳐도 와서 안기려 하지 않는 경우가 많다. 
  • 자기를 사랑하는 사람, 시끄러운 소리, 음식물 등을 싫어하고 고독하게 혼자 있기를 좋아하기도 한다. 
  • 부모나 다른 사람들을 좋아하는 대신 끈이나 열쇠 같은 생명이 없는 물건을 가지고 몇 개월에서 몇 년 동안 놀기를 더 좋아한다. 
  • 그 아이가 가지고 노는 것을 뺏으려고 하면 심하게 화를 내기도 한다. 동년배보다 말을 훨씬 늦게 배우든지 언어장애가 심하든지 말이 소통되지 않는 경우도 있다. 
  • 말을 전혀 하지 않거나 말할 때는 적절한 단어를 골라 쓸 줄 모르고 때로는 앵무새 같이 다른 사람의 말을 흉내 내기도 한다. 
  • 자폐증을 가진 아이는 강박관념이 강하기 때문에 가지고 놀던 장난감을 다른 사람이 조금만 만지거나 자신이 놓았던 순서를 조금만 바꿔 놓아도 몹시 화낼 수 있다. 
  • 전혀 의미가 없는 어떤 종류의 육체 운동을 기계적으로 계속 반복하기도 하고, 한 가지 음식물이나 특이한 음식물만 먹으려는 습성도 있다. 
  • 이 병이 있는 어떤 아이는 보통 사람들이 모르는 특수 단어, 구절, 숫자 등을 잘 욀 수도 있다. 
  • 자폐증 스펙트럼 장애가 있는 아이들의 46%가 학교에서 다른 학생들에 의해서 왕따를 당한다고 한다.소스: AMA Morning Round September 4, 2012

자폐증의 진단 

  • 병력, 증상 징후, 진찰소견 등을 종합해서 이 병을 진단한다. 
  • 이 병을 확진할 수 있는 타각적인 진찰소견을 발견할 수 없는 때가 많다. 
  • 일반적으로 소아청소년과 전문의, 소아 신경내과 전문의, 소아 정신과 전문의, 임상심리사 등이 한 개의 진단 치료 팀의 멤버가 되어 서로 협력해서 자폐증을 진단하고 치료한다. 그 외 여러 전문가들의 도움이 필요할 때도 있다.
  • 청력 검사, 시력검사, 혈중 납 농도, 염색체 검사, 분별적 기능적 뇌 MRI 검사 등을 해서 진단하는데 도움을 얻을 수 있다.

자폐증의 진단 기준

  •  다음 1, 2, 3 진단 기준 항목에 있는 증상 중 6개나 그 이상의 증상들이 있어야하고 
  • 6개 증상이나 그 이상의 증상 중 적어도 1 진단 기준 항목에서 1(하나) 증상, 2 와 3 진단 기준 항목에 있는 증상들 중에서 1(한 개)의 증상이 있으면 자폐증을 진단할 수 있다. 

  1. 진단 기준 항목 

  • 일상생활을 하는 중 상호관계에서 질적 장애가 다음 a, b, c 증상 목록 중 적어도 2개가 생길 수 있다.
a. 일상생활 중 상호관계를 정상적으로 맺는데 필요한 눈 맞추기(눈길 접촉), 얼굴 표현, 몸짓과 자세 행동 등과 같은 비언어적 행동에 두드러진 장애가  성장 발육에 적절치 않게 나타난다. 
b. 일상생활 중 동년배 또래들과 적절한 상호관계를 가질 수 있게 발육되지 않는다. 
c. 즐거움, 흥미, 성취 등에 관련된 행동을 할 때 자발적으로는 서로 나누어 가질 수 있는 욕구가 결핍되어 있다. 
d. 재미있는 것이 있을 때 그것을 다른 사람에게 보라고 하거나 가져다가 보이거나 손으로 가리키는 등 서로 재미를 행동으로 나누어 갖지 않는 상호작용 장애가 있다. 
e. 사회성 상호작용이나 정적 상호작용이 결핍되어 있다. 

  2. 진단 기준 항목 

  • 적어도 다음 a, b, c, d 증상 목록 중 1(하나)나 그 이상 의사소통에 질적 장애가 생길 수 있다.
a. 구어(입으로 하는 말)발육에 일부 지연되거나 아주 발육되지 않을 수 있다. 
b. 적절한 구어가 발육되어 있어도 대화를 시작하는 데 심한 장애가 있거나 대화를 정상적으로 계속 할 수 없다. 
c. 같은 말을 반복하거나 특이한 말을 이용해서 말하거나 
d. 여러 종류의 자발적 겉꾸밈 놀이, 사회성 모방놀이 등이 그 아이 성장 발육 이정표에 적절하지 않고 뒤떨어진다. 

  3. 진단 기준 항목 

  • 적어도 다음 a, b, c, d 증상 목록 중 1(하나)나 그 이상의 행동, 흥미, 활동 범위가 제한되어 있거나 그것을 반복하는 패턴이 생길 수 있다.
a. 반복되고 제한된 흥미 패턴이 비정상적으로 높은 강도로 나타나거나 그 흥미가 초점에 맞지 않고 그런 흥미에 도취되어 있다. 
b. 특이하고 비 기능적인 통상적 행동이나 의식적 행동에 변화가 생기지 않고 매달려있다. 
c. 손바닥이나 손가락을 펄럭거리거나 뒤틀거나 전 신체를 동일하게 반복적으로 복잡하게 운동하는 행동을 할 수 있다. 
d. 어떤 물건의 일부분에 지속적으로 골몰한다. 

다음 1, 2, 3 증상 항목에서 생후 3세 이전에 적어도 한 가지나 그 이상이 지연되거나 기능이 비정상적일 때 

① 사회성 상호작용 
② 사회적 의사소통 구어 
③ 상징적이거나 상상적인 놀이 

   레트 장애(Rett’s disorder)나 소아 파상적 장애보다 낫지 않을 때 

자폐증의 감별 진단

  • 레트 장애(Rett’s disorder), 
  • 여린 X 증후군(Fragile x syndrome), 
  • 란다우-클레프너증후군(Landau-Kleffner syndrome), 
  • 아동 붕괴 장애(Childhood disintegrative disorder), 
  • 확산성 발육 질환(Pervasive developmental disorder)과 감별 진단해야 한다. 

  1. 레트 장애 Rett’s disorder

  • 생후 5개월까지 두위의 크기가 정상이고 발육도 정상이다. 
  • 그 후 뇌에 이상이 생기고 손과 발의 운동에 이상이 생기고 상호 사교를 하지 않으려고 한다. 
  • 이 병은 여아에게서만 생긴다.
  •  유전성으로 생긴다고 믿는다. 
  • 때로는 4~5세 사이에 뇌 발육이 지연되면서 이 병이 생길 수 있다. 

  2. 여린 x 증후군 Fragile x syndrome

  • 성염색체 열성으로 유전되는 염색체 질병이다. 
  • 이병이 있는 아이들에게 지능장애, 언어장애, 행동장애, 고환 이상 등이 생길 수 있고 이 병이 있는 아이들의 7~8%에서 자폐증이 생길 수 있다. 

  3. 란다우-클레프너 증후군 Landau-Kleffner syndrome

  • 후천성 벙어리가 되고 
  • 전신 경련을 할 수 있고 
  • 뇌파검사가 비정상이다. 

  4. 결절성 경화증 Tuberous sclerosis

  5. 다운 증후군 Down’s syndrome

  6. 엔젤맨 증후군 Angelman’s syndrome

  7. 코르넬이나 데 랑게 증후군 Cornelia de Lange syndrome

  8. 스미스 램리 오피츠 증후군 Smith-Lemli-Opitz syndrome

  • 유전병이 있는 아이들에게도 자폐증이 생길 수 있다. 
  • 그런 병과 감별 진단해야 한다. 

  9. 선천성 대사 이상  Congenital metabolic disorders

  •  대사성 질환이 있는 아이들의 5%에게 자폐증 증상이 있을 수 있다. 
  • 따라서 때로는 대사이상에 관한 간기능 검사, 피와 소변에서 아민노산 검사를 해야 할 때도 있다.
  • 또 필요에 따라 뇌파검사도 하고 
  • 우드 램프(Wood’s lamp) 검사도 한다.

자폐증의 치료

  • 이 병은 완치되기 어려운 소아 신경 발육 장애 또는 행동장애 증후군의 일증이다. 
  • 일생 동안 계속 갖고 사는 경우가 대부분이다. 
  • 자폐증이 있는 아이들의 60~75%에서 인식 기술정도가 저능아 레벨에 속한다. 
  • 사회적 기술과 의사 소통기술 등을 특별히 교육(ASD)시키면 그들의 발육에 현저히 좋은 효과가 있다고 한다. 
  • TEACCH(Treatment and Education of Method of Education of Autistic, and Related Communication Handcaped Children)치료를 하면 치료효과가 좋다고 한다. 
  • 소아청소년과 전문의, 소아 정신과 전문의, 소아 신경내과 전문의, 임상심리사, 사회복지사, 언어전문가, 사회, 국가 등이 합심해서 한 팀이 되어 치료해 주어야 한다. 
  • 공격적이고 투쟁적 증상이나 성마른 증상, 분노발작, 파괴적인 행동, 활동과다증, 흥분하는 등 자폐증으로 생길 수 있는 증상을 Risperidone(Risperdal)제로 치료할 수 있도록 FDA가 최근 승인했다. 
  • 그러나 Clonidine, Bupropion, Atomoxetine, Guanfacine, Imipramine, Clomipramine, Fluvoxamine, Risperidone, Naltrexone 등의 약물 중 한 가지 약물을 선택해서 자폐증의 증상을 일부 의사들은 치료하기도 한다.

자폐증이 있는 아아들을 장기간 Melatonin으로 치료 하면 잠을 잘 잔다고 한다.

근데 이번엔 좀 심각한거 같습니다우리아이가 자폐가 아닌가 싶어서요.

아인 다섯 살이구요남자아입니다형젠 없구요집이 시골이라 변변한 친구도 없고 문화시설도 없습니다아인 그런대로 잘 지냈는데 올해 초완 다르게 많이 변해있었습니다.

그만한 아이 땐 억센 경향이 있다구들 하고 그래서 무심코 방치한 건 아닌지 아이에게 무슨 알 수없는 충격이 있었는지 알 수 없구요정신과 치룔 받아보라구 병원에선 그러더군요

아인 혼자서 노는 걸 좋아하구요잘 어울리지 못하구 고집도 엄청 셉니다.

남자아이라 그렇다구 어른들은 그렇지만 저 혼자서 놀 땐 뭐라고 하는지 알 수 없는 소리에 혼자 웃고 떠들기도 하구여 도무지 어른 말이라곤 듣질 않습니다.

그래서 아이 아빠는 매를 들기도 하구요 그때마다 맞는 아일 보면 속이 엄청 상합니다.

그리고 아인 유분증이 아직도 남아있습니다내년부터 유치원엘 보내려고 하는데 화장실도 못가는 아이 때문에 아이 기죽이고 따돌림 당할까 걱정입니다그럼 아이한테도 상처가 될텐데 걱정입니다지금 이순간도 아인 꽃밭에 올라가 있거나 모래더미 위에 올라가 놀고 있습니다.

안녕하세요좋은 질문을 해주셔서 감사합니다자녀의 나이성별과거 병력가족 병력진찰소견임상검사 등 자세한 정보가 더 많이 있을수록 더 좋은 답변을 드릴 수 있지만 주신 정보를 토대로 답변을 드리겠습니다.

여기는 설날 전야입니다교회 모임에 갔다가 돌아와서 답변을 드리는 것입니다.

새해에 복 많이 받으시고 가내 제위께서 평강하시기 바랍니다.

자녀의 문제에 관해서 저도 많이 걱정을 합니다.

첫째로 부탁드리고 싶은 것은 성경에 양을 지팡이로 인도한다는 말이 있지만 체벌로 자녀들을 양육하는 방법에 대해서는 저는 절대로 반대입니다.

체벌뿐만 아니라 벌로 자녀들을 양육하는 부모들이 많이 있지만 체벌로 훈련을 시키면 자녀들이 포악해지고나중에는 벌을 받을 때 받은 벌로 자기들의 잘못의 대가를 치른 줄 잘못 생각하고

때로는 벌에 대한 효과가 하나도 없고멸시를 당한 감을 갖고더구나 체벌은 자녀를 천하게 만들고자녀의 인간성을 짓밟고자녀에게 굴욕감을 줍니다그리고 부모에 대해 높은 인식을 갖지 않습니다.

또한 건전한 양심이 발달되지 않습니다체벌이 낳는 중요한 결과는 체벌을 받고 자랐던 아이들이 성인이 된 후에 자녀들을 체벌로 훈련시킵니다.

이런 체벌은 대를 이어 그 집안에 계속 될 수 있습니다.

우리는 부모로서 할 일을 아무리 다 잘 해도 자녀들에게 때때로 잘못을 저지릅니다.

우리 부모들은 완전한 부모도 없고 자식 역시 완전한 자녀도 있을 수 없습니다우리 부모들은 자녀들의 나이와 성장 정도에 따라 그들의 최선을 다 해서 올바르게 행동하기를 바랍니다.

Autism and Autism spectrum disorder

Overview of Autism

• not having or having trouble having normal social interactions with parents and others; • Delayed or undeveloped language development;

① Kanner’s Autism,

Causes of Infantile Autism (Autism) and Autism Spectrum Disorder

• 75-80% of children with Kanner’s autism have mental retardation, and children with Aspergall’s autism have less mental retardation.

• Most children with Kanner’s autism have a good memory, but some children with Aspergall’s autism sometimes have an exceptionally good memory.

• Most children with Kanner’s autism have obsessive-compulsive, repetitive behaviors, echolepticism, communication disorders, convulsive disorders, hyperactivity and attention-deficit disorder, eating pattern abnormalities, prefer to be alone, and He has a tendency to keep things going, is overreacting to stimuli, and has a good cognitive side. It is common for children of wealthy families to develop this disease.

• Most children with Asperger’s autism have social interaction disorder and are more likely to be boys, usually developing normally in infancy and sometimes by age 3 years.

• Children with autism differ in many ways from normal children, including:

• Children with autism are often lonely and alone because they have little or no close relationships with their parents or non-parents.

• There is a habit of mechanically repeating certain kinds of physical exercise that is meaningless and eating only one food or a specific food.

• Some children with this disease may memorize special words, phrases, and numbers that ordinary people do not know.

Diagnosis of Autism

• Diagnose this disease by synthesizing the medical history, symptom signs, and examination findings.

• In many cases, objective examination findings that can confirm this disease cannot be found.

Diagnostic Criteria for Autism

• You must have 6 or more of the symptoms listed in the following criteria 1, 2, and 3

• Autism can be diagnosed if at least 1 (one) of 6 symptoms or more symptoms is present in criteria, and 1

(1) of symptoms in 2 and 3 criteria is present.

1. Diagnostic criteria items

• Qualitative impairment in interrelationships during daily living may result in at least two of the following symptoms a, b, and c.

a. Significant impairments in nonverbal behaviors such as eye contact (eye contact), facial expressions, and gestures and postures, which are necessary for normal interaction in daily life, appear inappropriate for growth and development.

b. They are not developed to have appropriate interactions with their peers in their daily life.

c. They lack the desire to voluntarily share with each other when they engage in activities related to pleasure, interest, and achievement.

d. There is an interaction disorder in which people do not share the fun with each other, such as asking someone else to watch something interesting, bring it up, or point it with their hand. e. There is a lack of social or static interactions.

2. Diagnostic criteria items

• At least 1 (one) or more of the following symptoms a, b, c, d list may result in a qualitative impairment in communication.

a. There may be some delay in the development of oral (oral speech) or not at all. b. Even if adequate colloquial language is developed, there is a severe disability in starting a conversation or the inability to continue a conversation normally. c. repeating the same words or using unusual words d. Various kinds of voluntary decorative play, social imitation play, etc. are not appropriate for the child’s growth and development milestone and fall behind.

3. Diagnostic criteria items

• At least one (1) of the following symptoms a, b, c, or d list has a limited range of behaviors, interests, or activities, or a pattern of repeating them occurs.

a. Recurrent, restricted patterns of interest present with unusually high intensity, or the interest is out of focus and preoccupied with such interest.

b. Unusual, non-functional, normal or conscious behaviors hang without change.

c. It may involve flapping or twisting the palms or fingers, or performing the same repetitive and complex movements of the entire body.

d. Persistent preoccupation with a part of an object.

When at least one or more of the following symptoms 1, 2, and 3 are delayed or abnormal before 3 years of age

① social interaction

② Social communication spoken language

③ symbolic or imaginary play When not better than Rett’s disorder or pediatric destructive disorder

Differential Diagnosis of Autism

• Rett’s disorder;

• Fragile x syndrome;

• Landau-Kleffner syndrome;

• Childhood disintegrative disorder;

• It should be differentiated from a pervasive developmental disorder.

1. Rett’s disorder

• Until 5 months of age, the size of the head is normal and development is normal.

• After that, there is a problem in the brain, the movement of the hands and feet is abnormal, and there is an unwillingness to socialize.

• This disease occurs only in girls.

• Believed to be hereditary.

• Occasionally, between the ages of 4 and 5, brain development is delayed, causing the disease to develop.

2. Fragile x syndrome

• It is a chromosomal disease that is inherited as a sex chromosome recessive.

• Children with this disease may develop intellectual disabilities, speech disorders, behavioral disorders, and testicular abnormalities, and 7-8% of children with this disease may develop autism.

3. Landau-Kleffner syndrome

• Being acquired dumb • May have general convulsions

• EEG is abnormal.

4. Tuberous sclerosis

5. Down’s syndrome

6. Angelman’s syndrome

7. Cornelia de Lange syndrome

8. Smith-Lemli-Opitz syndrome

• Children with genetic disorders can also develop autism.

• Differential diagnoses should be made from such diseases.

9. Congenital metabolic disorders

• 5% of children with metabolic disorders may have symptoms of autism.

• Therefore, sometimes it is necessary to test for liver function for metabolic abnormalities and to test for aminic acid in blood and urine.
