정신 지체(정신 발육지체/지능 장애/정신박약)의 개요

  • 지능 지수(IQ)가 정상보다 상당히 낮고 
  • 적응행동(행동적응/Adaptive behavior)과 인지 기능에 상당한 장애가 있고 
  • 인간 기본 생존 자기 관리, 
  • 지역사회 생존 기술, 
  • 가정과 사회생활,
  • 학교 학습 등을 정상적으로 수행할 수 없는 상태를 정신지체, 정신 발육 지체, 지능 장애, 또는 정신박약이라 한다. 
  • 정신 지체는 생후 0~18세 동안 성장 발육하는 소아청소년들에게 나타날 수 있다. 
  • 정신 지체가 있는 소아청소년들은 전화를 적절히 하고 받을 수도 없고, 
  • 돈을 적절히 쓰고 환전할 수 없고 
  • 공중 교통수단을 적절히 이용할 수 없다. 
  • 전 인구의 약 3%에서 정신 지체가 있다. 
  • 어떤 사람들은 Mental retardation이라고 말하는 대신 “Idiot”, “Imbecile”, 또는 “Moron” 이라  한다. 
  • 전문가들 대부분은 Mental retardation이란 용어는 모욕적인 용어가 될 수 있기 때문에 Mental Retardation이란 병명 대신 다음과 같은 용어를 쓰자는 것이 요즘이다.
  • “Mentally challenged”, 
  • “Intellectual disability”, 
  • “Mental impairment”,
  • “Developmental disability, 
  • “Intellectual and Developmental Disabilities,”
  • “Mental handicap”, 
  • “Mental deficiency”

중간 정도 정신지체
Moderate mental retardation
IQ 35-49 초등학교 1-2학년 정도로 읽을 수 있다. 도움없이 옷을 입고 벗을 수 있고 변기를 사용할 수 있고 음식을 만들어 먹을 수 있다 자고 먹고 살면서 고용될 가능성이 있다. 5-8세 제한적으로
심한 정신지체
Severe mental retardation
IQ 20-34 읽고 쓰는 것을 거의 할 수 없다. 대소변을 가리는 훈련을 받을 수 있다. 도움을 받아 옷을 입고 벗을 수 있다. 사인을 할 수 있다. 자고먹고 살면서 고용될 수 있다. 3-5세 광범위 하게
아주 심한 정신지체
Profound mental retardation
IQ 20 이하 전혀 읽고 쓸 수 없다. 때로는 대소변 훈련을 받을 수 있다. 도움받아 옷을 입고 벗을 수 있다. 거의 말은 할 수 없다. 고용될 가능성이 없다. 3세 이하 현실성이 없다.

 

정신 지체(정신 발육지체/지능 장애/정신박약)의 원인

① 유전

② 출생 전 생긴 태아 감염병 또는 출생 후 생긴 감염병, 

③ 염색체 이상, 

④ 외상,

⑤ 안전사고, 

⑥ 중독 

⑦ 단백질 대사 이상

⑧ 아미노산 대사 이상(Amino Acid disorders)

등으로 정신 지체가 생길 수 있다. 

때로는 정신 지체 부모들의 자녀들에게도 정신 지체아가 생길 수 있다.  

정신 지체가 있는 증후군과 질환 

  • 소토스 증후군(Sotos-syndrome), 
  • 클라인펠터 증후군, 
  • 취약 X 증후군, 
  • Phelan-McDermid syndrome (22q13del), 
  • 모왓-윌슨 증후군, 
  • 다운 증후군 등 삼 염색체증후군, 
  • X 염색체 이상, 
  • Y 염색체 이상, 
  • 48, XXXX 증후군, 
  • 49, XXXXX 증후군, 
  • 47, XYY 증후군, 
  • 49, XXXXY 증후군, 
  • 49, XYYYY 증후군 등 염색체 이상이 있거나 
  • 태아 알코올 증후군, 
  • 주산기 산소 결핍증, 
  • 분만 중 뇌 손상이 있거나, 
  • 출생 전 태아가 사이토메갈리(거대세포)바이러스 감염병, 헤르페스바이러스 감염병, 태아 풍진 증후군, 태아 톡소풀라시모시스 감염병, 매독 등 TORCH, 
  • 출생 후 면역결핍바이러스 감염병, 백일해, 홍역, 뇌염, 뇌막염 등을 앓았거나,
  • 코케인, 알코올, 방사능, 납, 또는 수은 등에 중독되거나,
  • 태아 영양실조, 
  • 태반기능 부전증, 
  • 극소 미숙 신생아와 저체중 신생아, 
  • 저 산소증, 
  • 두개강 내 출혈, 
  • 신생아 저혈당증, 
  • 고 빌리루빈증이 있거나, 
  • 요도도 결핍증(아이다인 결핍증)등이 있거나
  • 신경섬유종증, 
  • 신경피부 질환, 
  • 선천성 갑상성기능 저하증, 
  • 윌리암스 증후군, 
  • 페닐키톤뇨증 (PKU), 
  • 프라더 윌리 증후군 등이 있거나,
  • 뇌 외상, 
  • 질식, 
  • 익수 사고가 있거나 
  • 고 소듐증이 있거나
  • 영양실조에 걸리거나
  • 부모에게 정신병이 있거나, 
  • 부모가 마약에 중독되거나, 
  • 극심하게 가난하거나 
  • 부모로부터 사랑을 못 받으면 정신 지체가 생길 수 있다.

정신 지체(정신 발육지체/지능 장애/ 정신박약)의 증상 징후

  • 원인에 따라 증상 징후가 차이 나고 
  • 정신 지체의 정도에 따라 다를 수 있지만 태어나서부터 발육지연이 생길 수 있다. 
  • 머리를 들고 엎치고 앉고 기고서는 발육지연이 있을 수 있다. 
  • 언어 발육과 구어 발육 지연이 생기고 
  • 기억력이 부족하고 
  • 사회 규칙과 룰을 따르지 못하고, 
  • 사회생활을 하는 규칙을 적절히 배울 수 없다. 
  • 어떤 기술이든 배우는 데 어려움이 있고, 
  • 세수를 한다든지, 밥을 먹는다든지, 대변보고 뒤를 닦는다든지 인간 기본 생존 자기관리를 나이에 따라 적절히 할 수 없다. 
  • 사회생활에서 해서는 안 될 행동을 하는 경향이 있다. 
  • 자제를 잘 하지 못한다. 
  • 제한할 것을 제한하지 않을 수 있다. 
  • 일반적으로 지능이 낮다. 

정신 지체(정신 발육지체/지능 장애/정신박약)의 진단

  • 병력, 가족력, 증상 징후, 검진, IQ 검사, 적응행동(행동적응/Adaptive behavior) 검사 등을 해서 진단한다.

정신 지체(정신 발육지체/지능 장애/정신박약)의 치료

  • 정신 지체는 우울증이나 정신 분열증과 다르다. 
  • 정신 지체의 원인과 종류에 따라 약물로 치료하고 교육, 직업 훈련 등으로 치료한다. 
  • 가정, 부모, 보호자, 의사, 교육계, 의료계의 각종 전문가, 사회 복지사, 국가, 사회, 지역사회 등이 한 팀이 돼서 정신 지체아들을 도와주는 것이 이상적이다. 
  • 국가 사회 영리 단체나 비영리의 단체, 종교 단체에서 정신 지체아들을 도와주기도 한다. 

Mental retardation (intellectual disability)

Overview of Mental Retardation

• Your IQ is significantly lower than normal and

• Significant impairments in adaptive behavior and cognitive functions;

• Human basic survival self-management;

• community survival skills;

• family and social life;

• A condition in which students cannot normally perform school studies, etc., is called mental retardation, mental retardation, intellectual disability, or mental retardation.

• Mental retardation may occur in children and adolescents who grow and develop between the ages of 0 and 18 years of age.

• Children and adolescents with mental retardation are unable to make and receive phone calls properly;

• You cannot spend money properly and exchange it;

• Lack of adequate use of public transport.

• About 3% of the population has mental retardation.

• Some people say “Idiot”, “Imbecile”, or “Moron” instead of Mental retardation.

• Most experts say that the term mental retardation can be an offensive term, so instead of the disease name, the following term is used these days.

• “Mentally challenged”,

• “Intellectual disability”;

• “Mental impairment”;

• “Developmental disability;

• “Intellectual and Developmental Disabilities,”

• “Mental handicap”,

• “Mental deficiency”

• “Learning disability”; • “Learning difficulties”.

Causes of mental retardation 

① hereditary

② Fetal infectious disease that occurred before birth or infectious disease that occurred after birth;

③ chromosomal abnormalities,

④ trauma,

⑤ safety accidents; ⑥ Addiction

⑦ Abnormal protein metabolism

⑧ Amino Acid disorders Mental retardation may result. Sometimes children of mentally retarded parents can also developmentally retarded children.

Syndromes and diseases with mental retardation

• Sotos-syndrome;

• Klinefelter’s syndrome;

• Fragile X Syndrome;

• Phelan-McDermid syndrome (22q13del),

• Mowat-Wilson syndrome;

• Trisomy syndrome, such as Down syndrome;

• X chromosome abnormalities;

• Y chromosome abnormalities;

• 48, XXXX Syndrome;

• 49, XXXXX syndrome;

• 47, XYY Syndrome;

• 49, XXXXY Syndrome;

• If there is a chromosomal abnormality such as 49, XYYYY syndrome, or

• Fetal Alcohol Syndrome;

• perinatal oxygen starvation; • you have brain damage during delivery;

• TORCH, such as cytomegalic (cytomegalovirus) virus infection, herpes virus infection, fetal rubella syndrome, fetal Toxofulasimosis infection, syphilis, etc.

• Have suffered from an immunodeficiency virus infection, whooping cough, measles, encephalitis, or meningitis after birth;

• poisoned with cocaine, alcohol, radiation, lead, or mercury;

• fetal malnutrition; • placental insufficiency;

• Very premature and low birth weight newborns;

• hypoxia,

• intracranial hemorrhage;

• neonatal hypoglycemia;

• have high bilirubinosis;

• You have urethral deficiency (aidain deficiency), or

• Neurofibromatosis;

• neurodermatologic diseases;

• Congenital hypothyroidism;

• Williams syndrome;

• Phenylchitonuria (PKU);

• You have Prader-Willi Syndrome, • brain trauma;

• Choking;

• There is a drowning accident;

• have hypersodium or

• are malnourished;

• The parent has a mental illness;

• the parent is addicted to drugs;

• extremely poor or

• Lack of love from parents can lead to mental retardation.

Signs, symptoms of mental retardation

• Symptoms and signs are different depending on the cause.

• It may be different depending on the degree of mental retardation, but developmental delay may occur from birth.

• There may be developmental delays when sitting on your stomach and crawling with your head raised.

• delays in speech and speech development;

• lack of memory

• Failure to follow social rules and rules;

• Inability to properly learn the rules of social life.

• have difficulty learning any skill;

• It is impossible to properly manage basic human survival self-management such as washing face, eating rice, or wiping after a bowel movement according to age.

• Tendency to engage in behaviors that should not be done in social life.

• Poor self-control.

• Restrictions may not be restricted.

• Generally low intelligence.

Diagnosis of mental retardation

• Diagnosis is made through medical history, family history, symptom signs, examination, IQ test, and adaptive behavior test.

Treatment of mental retardation

• Mental retardation is different from depression or schizophrenia.

• Depending on the cause and type of mental retardation, it is treated with drugs and is treated with education and vocational training.

• Ideally, families, parents, guardians, doctors, educators, various experts in the medical field, social workers, the state, society, and the community should come together as a team to help mentally retarded children.

• National social for-profit organizations, non-profit organizations, and religious organizations help children with mental disabilities.
