틱증(경련성 불수의 운동)의 개요

  • 신체의 여러 종류의 근육 중 일부 근육이나 여러 근육들이l 갑자기 일시적으로 경련하는 병을 틱증이라 한다. 
  • 틱증은 불수의적으로 무의식적으로 생긴다. 
  • 틱증이 발작되면 신체의 일부의 근육이나 여러 부위의 근육들이 잠시 동안 국소적으로 경련하다가 자연적으로 그친다. 
  • 틱증 발작은 긴장하거나 불안할 때 더 심하게 유발되는 것이 보통이다. 

틱증(경련성 불수의 운동)의 종류

  • 틱증을

① 단순 틱증(Simple tics),

② 투렛 증후군(Tourette syndrome), 

③ 운동성 만성 틱증(Chronic motor tics)으로 3분한다. 

  • 단순 턱증은 예후가 좋다. 
  • 투렛 증후군은 일반적으로 예후가 좋지 않다. 
  • 여기서는 단순 틱증과 운동성 만성 틱증에 관해 주로 설명한다. 

  1. 단순 틱증 

   1). 단순 틱증의 개요

  • 신체의 일부의 근육이나 신체의 여러 부위의 근육들이 무의식적으로 갑자기 경련되다가 잠시 후에 갑자기 경련이 끝나는 틱증을 단순 틱증 또는 습관성 틱증이라고 한다. 
  • 단순 틱증은 소아청소년들에게 상당히 흔하게 유발된다. 
  • 영유아들이나 학동기 아이들, 사춘기 아이들, 성인들에게도 생길 수 있다. 
  • 6∼10세 학령기 아이들에게 더 잘 생긴다. 
  • 단순 틱증은 학령 아이들의 12∼24%에게 생긴다. 
  • 대부분의 단순 틱증은 처음 발생된 후 1~2개월 동안 생기다가 안생기다가 길게는 1~2년이 지나면 자연적으로 낫는다. 

   2). 단순 틱증의 증상 징후

  • 단순 틱증 발작이 유발되면 신체의 한 개의 근육이나 여러 개의 근육이 제멋대로 수위적으로 경련하기도 한다. 
  • 틱증 발작이 유발될 것이라고 예측할 수 없다. 
  • 갑자기 유발되서 잠시 동안 계속 경련되다가 갑자기 끝나는 것이 보통이다. 
  • 이런 틱증이 반복될 수 있다. 
  • 틱증 발작이 몇 시간 동안이나 며칠 동안 유발되었다가 또다시 발작이 유발될 수 있다(p.00 히스테리 참조).
  • 덜 불안하거나 스트레스나 긴장이 풀리거나, 어떤 일에 재미 들어 정신 팔렸을 때는 틱증이 덜 발작되거나 얼마 동안 발작되지 않을 수 있다. 
  • 틱증은 깨어 있는 동안 어느 때든지 유발되며 틱증 발작이 유발 될 때 마음대로 중지할 수 없다. 
  • 틱증 발작을 의식적으로 중지하려고 할 때는 더 심해질 수 있다. 
  • 단순 틱증 발작이 유발되면 눈을 깜박거리거나 얼굴 근육의 일부가 씰룩거릴 수 있다. 
  • 어깨를 으쓱으쓱 추켜올리거나, 드물게는 머리, 전신 또는 팔다리 등을 제멋대로 움직일 수도 있다. 
  • 틱증이 신체의 한 부위의 근육에만 생길 수도 있고 신체의 여러 부위에 있는 근육에 동시 생길 수도 있다. 
  • 목구멍 속에 무엇이 걸려서 그것을 뱉어내려고 하는 것같이 켁 켁 목구멍소리를 내는 틱증발작을 할 수 있다. 
  • 으흠 하는 목구멍소리를 내기도 한다. 
  • 이런 종류의 틱증 발작이 유발 되면 목구멍 속에 무엇이 걸려서 그런 목구멍소리를 내는 줄로 잘못 생각하기도 한다. 
  • 이런 틱증 발작을 인두 강을 깨끗이 하려는 틱증(Throat clearing)이라고도 한다. 인두를 깨끗이 하려는 틱증 발작은 천식이나 알레르기성 비염이 있는 아이들에게 더 잘 유발된다. 

   3). 단순 틱증의 진단 

  • 병력, 증상 징후와 진찰소견 등을 종합하여 진단한다. 
  • 히스테리와 감별 진단해야 한다.

   4). 단순 틱증의 치료 

  • 단순 턱증의 예후는 좋다.
  • 대부분의 단순 틱증발작은 잠시 동안 유발됐다가 자연히 없어지기도 하고, 2개월 내지 1~2년 을 거쳐 유발되었다가 얼마동안은 유발되지 안했다가 더 이상 유발되지 않을 수 있다. 
  • 거의 모든 단순 틱증은 특별한 치료를 받을 필요가 없이 자연히 낫는다. 
  • 그러나 긴장하거나 근심 걱정을 많이 하면서 스트레스를 많이 받고 있으면 소아 정신과 전문의이나 소아 신경내과 전문의 등의 치료를 받아야 한다. 
  • 자유 시간을 충분히 갖도록 하고 긴장을 풀어 주고 재미있는 시간을 더 많이 갖도록 해야 한다. 그 동안 했던 공부, 과외활동 및 일상생활의 패턴을 재평가하여 환아의 능력과 적성에 맞도록 조절해야 한다. 
  • 단순 틱증이 발작될 때 놀리거나 비난해서는 절대로 안 되며 그 자녀의 앞에서 다른 형제자매들과 비교해서도 안 된다. 
  • 공부를 잘 하라고 압력을 주어서도 안 된다. 
  • 틱증 발작이 계속 심하게 유발되고 학교 공부나 일상생활을 하는데 지장 주면 적극적으로 치료받아야 한다. 
  • 일부의 근육이 틱증 발작 유발로 경련할 때 놀리거나 “그렇게 하지 말라”는 식으로 충고하거나 꾸짖으면 틱증 발작이 멈추기보다 오히려 더 심하게 경련할 수 있다. 
  • 가정불화가 있으면 그것도 해결해야 한다. 
  • 화가 날 때 화를 가슴 속 깊이 품고 억누르지 말고 화를 적절히 표출하도록 훈련을 시켜야 한다. 
  • 틱증이 심하게 발작되면 심리요법으로 치료 할 수 있다.

  2. 운동성 만성 틱증

  • 대부분의 운동성 만성 틱증 발작은 한번 유발되면 그 후 일생을 두고 계속 유발되는 것이 보통이다.

Tics Overview of Tics

• A disease in which some or several muscles suddenly and temporarily cramps among various types of muscles in the body is called tics. \

• Tics occur involuntarily and unconsciously.

• When a tic is attacked, the muscles of a part of the body or muscles of various parts of the body spasm locally for a short time and then spontaneously stop.

• Tic attacks are usually more severe when nervous or anxious.

Types of tics

• Tic

① Simple tics,

② Tourette syndrome,

③ chronic motor tics.

• Simple jaw syndrome has a good prognosis.

• Tourette’s syndrome generally has a poor prognosis.

• Here, we mainly focus on simple tics and motor chronic tics.

1. Simple Tic

1). Overview of  Simple Tic

• A tic disorder in which the muscles of a part of the body or muscles of various parts of the body are involuntarily convulsed and then the convulsions end abruptly after a while is called simple tics or habitual tics.

• Simple tics are quite common in children and adolescents.

• It can also occur in infants, school-aged children, adolescents, and adults.

• It is more common in 6-10-year-old school-age children.

• Simple tics occur in 12-24% of school-age children.

• Most cases of simple tics appear for 1 to 2 months after they first appear, and then disappear spontaneously after 1 to 2 years.

2). Signs, symptoms of simple tics

• When a simple tic attack is triggered, one muscle or several muscles in the body may spontaneously and voluntarily convulse.

• It cannot be predicted that tic attacks will occur.

• It is usually triggered suddenly, convulsing for a short time and ending abruptly.

• These tics may recur.

• A tic attack may last for hours or days and then trigger again (see Hysteria).

• If you are less anxious, stressed or relaxed, or if you are distracted by something, you may have less tics or you may not have seizures for a while.

• Tics can occur at any time while awake and cannot be stopped when a tic attack is triggered.

• It can get worse when you consciously try to stop a tic attack.

• When a simple tic attack is triggered, you may blink your eyes or twitch in some of your facial muscles.

• You may shrug your shoulders and, rarely, move your head, whole body, or limbs at will.

• Tics can only affect a muscle in one part of the body or it can affect muscles in multiple parts of the body.

• You may have a tic-tick that sounds like something is stuck in your throat and you are trying to spit it out.

• You may also make a muffled throat sound.

• When this type of tic attack is triggered, it is sometimes mistakenly thought that something is stuck in the throat that is causing the throat sound.

• These tic attacks are also called throat clearing. Tic attacks to clear the pharynx are more likely to occur in children with asthma or allergic rhinitis.

3). Diagnosis of simple tics

• Diagnosis is made by combining medical history, symptoms, signs, and examination findings. • It should be diagnosed differently from hysteria.

4). treatment of simple tics

• The prognosis of simple jaw syndrome is good.

• Most simple tic evoked seizures are triggered for a short time and then go away spontaneously.

• Almost all tics simply get better without any special treatment.

• I am a mother of a 7-year-old boy.
