뇌전증의 증상․징후

■ 소아 결신 발작 뇌전증(Childhood absence epilepsy/Petit mal epilepsy)

▪ 소아 뇌전증의 2~4%가 소아 결신 발작 뇌전증이다.

▪ 3~10세경 이 뇌전증이 발병한다.

▪ 잠깐 동안 공백 상태에서 멍하는 징후가 생긴다.

▪ 특이한 뇌파 검사 결과가 나타난다.

▪ 이 뇌전증의 40%는 자연히 완화되거나, 10대가 되면 완화된다.

■ 연소성 근(육)간대 뇌전증 (Juvenile myoclonic epilepsy)

▪ 아주 건강한 10대 아이들에게 이 형의 뇌전증이 시작할 수 있다.

▪ 팔이 잠간 동안 움찔움찔하는 증상이 처음에 나타나고

▪ 아침에 일어나서 전신 발작을 할 수 있다.

▪ 항뇌전증 약물로 잘 치료된다.

▪ 대개, 성장하면서 자연히 치료되지 않는 것이 보통이다.

■ 양성 롤랜딕 뇌전증(Benign rolandic epilepsy/Benign partial epilepsy)

▪ 3~13세 아이들에게 나타나는 뇌전증이다.

▪ 침을 흘리고 말을 할 수 없는 등의 얼굴에 관련된 증상․징후가 나타난다.

▪ 이런 증상․징후가 나타난 후 전신 경련을 할 수도 있다. 이런 발작은 주로 밤에 일어난다.

▪ 그 외에는 건강하다.

▪ 95%가 15세가 되기 전에 더 이상 발작을 하지 않는다.

■ 영아 연축 (Infantile spasm)

▪ 발작 인식과 발작 1차 응급 처치 도표 5 중 5를 참조한다.

▪ 갑자기 근(육)간대 발작이 군집으로 빨리 생기고

▪ 팔을 뻗치거나 구부리고, 움찔움찔하고, 몸을 굽히면서 발작을 한다.

▪ 뇌파 검사에서 특이한 결과가 나타난다.

▪ 1세 전에 발병한다.

▪ 이 뇌전증이 있는 아이들에게 발육기형이 생기고 다른 형의 뇌전증으로 발전할 수 있다.

▪ 스테로이드와 항뇌전증 약물로 치료한다.

■ 레녹스 가스토 증후군 (Lennox-Gastaut syndrome)

▪ 소발작(결신 발작), 긴장발작 등 여러 형의 발작이 같이 생길 수 있고

▪ 치료하기 어려운 뇌전증이다.

▪ 신체가 빳빳이 단단해질 수 있고

▪ 수면 발작이 생기기도 하고

▪ 탈력 발작, 경련 발작 등이 생길 수도 있고

▪ 특이한 뇌파 검사 결과가 나타난다.

▪ 1~5세경에 발병한다.

▪ 발육지연이 생기고 정신지체아가 된다.

■ 라스무센 증후군 (Rasmussen’s syndrome)

▪ 한쪽 뇌의 이상으로 생긴다.

■ 란다우–클레프프너 증후군(Landau-Kleffner syndrome)

▪ 언어기능을 잃는다.

뇌전증의 증상․징후 Symptoms and signs of epilepsy 

■ 소아 결신 발작 뇌전증(Childhood absence epilepsy/Petit mal epilepsy)

▪Children’s absence seizure epilepsy accounts for 2-4% of epilepsy in children. ▪ Around the age of 3 to 10, this epilepsy develops. ▪ Signs of dizziness after a brief pause. ▪ Unusual EEG test results appear. ▪ 40% of these epilepsy cases resolve spontaneously, or when they reach their teens.

■ 연소성 근(육)간대 뇌전증 (Juvenile myoclonic epilepsy)

▪ This type of epilepsy can begin in very healthy teenagers. ▪ The first symptoms of twitching while the arm is asleep ▪ You may have generalized seizures when you wake up in the morning. ▪ It is well treated with anti-epileptic drugs. ▪ Usually, it does not heal spontaneously as you grow up.

■ 양성 롤랜딕 뇌전증(Benign rolandic epilepsy/Benign partial epilepsy)

▪ It is an epilepsy that occurs in children aged 3 to 13 years. ▪ Symptoms and signs related to the face, such as drooling and inability to speak, appear. ▪ You may have generalized convulsions after these symptoms/signs appear. These seizures usually occur at night. ▪ Otherwise, it is healthy. ▪ 95% no longer have seizures before the age of 15.

■ 영아 연축 (Infantile spasm)

▪ Seizure Recognition and Seizure First Aid See Table 5 of 5. ▪ Sudden, rapidly clustered myoclonic seizures ▪ Seizures by stretching or bending the arm, twitching, and bending. ▪ The EEG test shows unusual results. ▪ Onset before 1 year of age. ▪ Children with this type of epilepsy may develop developmental abnormalities and develop into other types of epilepsy. ▪ Treat with steroids and anti-epileptic drugs.

■ 레녹스 가스토 증후군 (Lennox-Gastaut syndrome)

▪ Several types of seizures, such as small seizures (absence seizures) and tonic seizures, may occur together. ▪ Epilepsy is difficult to treat. ▪ The body can become stiff ▪ You may have narcolepsy ▪ Catalytic seizures, convulsive seizures, etc. may occur. ▪ Unusual EEG test results appear. ▪ It develops around the age of 1 to 5 years. ▪ Growth retardation occurs and the child becomes mentally retarded.

■ 라스무센 증후군 (Rasmussen’s syndrome)

▪ Caused by abnormalities in one brain

■ 란다우–클레프프너 증후군(Landau-Kleffner syndrome)

▪ loss of language skills
